Kim goes into labor and J.D. meets her at the hospital where he volunteers to serve as her anesthesiologist. Meanwhile, Turk tries to find someone to finish his videogame with and learns that Dr. Kelso is living in the hospital after being kicked out of his house. The Janitor stops by and offers to play with Turk.
Kim asks J.D. how he fells about "us." He's temporarily saved when Dr. Cox comes in with his daughter looking for the pediatrician to give her a needed shot - he doesn't want to do it himself because he doesn't want his daughter associating him with pain. When Cox leaves, Kim says "I love you" to J.D., and he responds with, "cool."
Cox refuses to let the interns give his daughter the shot. In another room, the Janitor plays the videogame with Turk, pausing to say a few words for a slain space goblin. In Kim's room, she continues to hammer J.D. about the "love" issue, and he says he hopes to fall in love with her someday. She flips out and realizes that they are done as a couple.
Kim kicks J.D. out of the room and calls Elliot for support. In the break room, the Janitor gives up on the game and Carla takes his place, admitting she got addicted to the game when their daughter was sick. In the hall, Cox finds an upset J.D. and asks him to give his daughter a shot.
Cox counsels J.D. on being a good parent even if he and Kim aren't together. J.D. realizes that parenting is about sacrifice and runs into the room to watch his child being born. Dr. Cox relents and lets the interns give his daughter her shot. Meanwhile, Kim gives birth and J.D. holds his son for the first time.
